Adult parental support / Leaders forms to download
This is the list of forms that you will need to work with 1st Victoria Sea scouts. It appears daunting but it isn't. You will need two passport photos and a trip to the police station or other signatory to have your original documents signed. Most forms are available at VicScouts if the links below do not work.
1. Working with Children form
This form is available from the post office and scout hall. You will need a standard passport photo and 100 points of ID (see below) . There is no cost (other than the photos) and should take about 15 minutes. The details you will require to fill the form in are...

2. Victorian Police Check
The Police check is required for all overnight activities and is also free. This is returned to the Scout Association via the Group Leader. You will need 100 points of id (use the same as the WWC) and a passport photo. See the instructions file below for further details.

3. Adult Leader/Adult Helper/Fellowship Member (A1 form)
This form is for all new leaders and assistants and can be downloaded here
4. Mutual Agreement form
This form is for all new leaders and assistants and only needs to be signed. It can be found here.
5a. Adult Support Personnel ApplicationThis form is for adults who wish to become assistants and is here. We would prefer you to go down the uniformed 5b. route
5b. Application for a Uniformed Appointment (A2 form)
This form is for all new leaders only and is located here.
6. Available nights for training
There is no form for this but we would like to know which nights suit you to be trained. Obviously we don't hold you to this, it just makes it easier

The 100 points of identity requirements
You will need to present the original copies of Passports, bank statements, Drivers license etc and have these signed off. Details are available at Department of Justice website and is outlined below as a reference but may change slightly.

- Foreign passport 70
- Australian Driver Licence 40
- Victorian Firearms Licence or equivalent issued in other Australian state or territory 40
Category B Points
- Australian Birth Certificate 70
- Travel document with same characteristics as a passport or a current visa within the meaning of section 29 of the Migration Act 1958 of the Commonwealth 70
- Certificate of evidence of Australian residence status 70
- Australian Citizenship Certificate 70
- Card issued by the Commonwealth Department of Veteran Affairs 40
- Centrelink card (containing a reference number) 40
- Identity card issued by an Australian tertiary educational institution 40
- Private security individual operator licence 40
- Medicare Card 25
- Change of name certificate issued by a registry of births, deaths and marriages of an Australian state or territory 25
- Marriage Certificate issued by an agency of an Australian state or territory 25
- A credit or account card issued by a bank, credit union or other deposit taking institution within the meaning of the Banking Act of the
- Commonwealth 25
- Taxation Assessment Notice 25
- Motor Vehicle Registration 25
- Superannuation Statement 25
- Any document from Category A not already produced to meet the requirements of that category
- * To be rated points in accordance with Category A
- * Any other document that in the opinion of the Secretary is of a similar character to the documents previously listed in Category B. These include: 25

- • Marriage Certificate issued by an authorised minister of religion or marriage celebrant in an Australian state or territory 25
- • Australian Driver Learner Permit 25
Category C Points
- Utilities notice (water rates, electricity, gas or phone account) 25
- Rental contract or receipt issued by landlord, agent or owner 25
- Financial institution statement for current account 25
- Electoral enrolment for current address 25
- Any document from category A or B that has not been produced to meet the requirements of those categories and that states the current address of the applicant
- ** To be rated points in accordance with the point value from the relevant category
- ** Any other document that in the opinion of the Secretary is of a similar character to the documents previously listed in category C. This includes: 25
